How do I get access to my records?
You have rights under the Data Protection Act to access your medical records or authorise a representative to do so. Information may be withheld if we believe it could harm to your physical or mental health. We do not have to tell you that information has been withheld.
We require a written request: please print out and complete the form below, and bring it in to one of our practice sites or post it to Business Manager, Diamond Medical Group, Rutland Lodge, Scott Hall Road, Leeds LS7 3DR. Please note there is a charge (details on the form).
